I believe that a travel card allows you to use all London buses, even those outside your zones, so you could catch a bus to zone 2 and then get a train.

Could be that. Certainly, the doors close to the stairs were being used regularly as those just arriving jumped on board assuming the train was just about to leave. And of course a few of those already on board were getting off from time to time as the delay continued.

Time restrictions vary from ticket to ticket. Each off-peak ticket has a restriction code. Assuming your ticket is this one it has a restriction code of B3, which means it is not valid before 9am, but there are no afternoon peak restrictions (see https://www.nationalrail.co.uk/b3)
I've had a few e-tickets using the Avanti App and it was a simple Aztec code - no activation required. I just showed it on request and it was scanned. I don't know why activation is mentioned on the Avanti website. Maybe it actually means download to your device?
