Anyone could just make up a number. Underground ticket machines don’t have access to the Railcard database (or season ticket holder database in the event of a Gold Card being held).

It’s not exactly a hardship to get a member of staff to load the discount. I’ve got to say that in my experience (and despite fears to the contrary) it’s got far easier than in the days when you hadn’t to go to an Underground ticket office.
Not to mention the fact that many railcards will have the same number.

When I got my Disabled Railcard added last year onto my Oyster at Kings Cross, they didn't even bother about asking if it was registered. The LU member of staff added it on without any questions.

I'm certain they didn't even enter the railcard number either.

You can register the Oyster card when adding the Railcard. It does not need to be done online, nor does the Oyster card need to have been used for a journey. It can be brand new.
