The XX.02 services are the former Ipswich semi-fasts extended to Norwich.
No they are Intercity services that previously left at XX:00. The stopping EMU in these hours is still there & leaves two minutes later at XX:04.

Glasgow Queen Street to Edinburgh, regular departures at XX:00, XX:15, XX:27, XX:30, XX:45, XX:57- of those the 27 & 57 go via Cumbernauld and aren't shuttles, but fulfill the criteria and may be the most consistent schedules of this kind. And that before we include the low level platforms which also have four trains an hour to Edinburgh (although don't originate there).

Edinburgh features quite a lot on the Queen Street departure board.

Waterloo has many departures to Waterloo within a few minutes of departures from Waterloo to Waterloo! Sorry - I couldn't resist this one (the 'roundabout' service).

There are 2 departures to York at 1255 from different platforms at Church Fenton. I know they come from different routes but how are they able to schedule them for exactly the same time?

It's four tracks/platforms at Church Fenton and four tracks Northwards, the TPE train runs straight through to York on the fasts, the Northern stopper runs on the slows stopping at Ulleskelf (which only has platforms of the slow lines). No reason they can't leave at the same time.

Edit: I realise use of 'fast/slow', 'main/relief' etc. varies and I may not be using them correctly in this case; for these purposes I am taking the 'slows' to be the two tracks with platforms at Ulleskelf and the fasts as the non-platform lines.

Two departures at 09.13 from Liverpool Lime Street to Chester, Merseyrail via Birkenhead and TfW via Runcorn and Halton Curve.

In the current proposals for the December 2019 timetable, as discussed in a different thread, there are two Edinburgh (Waverley) - Manchester Airport services at 08:11 operated by TransPennineExpress, one via Newcastle-Leeds and the other via Carlisle-Preston (see eg 7th January 2020 on RealTimeTrains). Whether this will actually be the case when the timetable is confirmed is a different matter; I can see one being re-timed by a minute to avoid confusion!

Late on a Sunday night (so not a peak time), there are departures from Kings Cross to York at 22:00 and 22:05 (the former goes through to Newcastle).

On Sunday mornings there are two services from Edinburgh Waverley to Aberdeen within 3 minutes of each other - LNER at 09.10 and Scotrail at 09.13.

I'm not entirely sure about other days but on Sundays, the 22:40ish XC and 22:48 EMT services from Derby to Leeds leave Sheffield within like 4 minutes of each other. Also, they arrive at Leeds at 00:08 (EMT) and 00:09 (XC)!

On weekdays and I guess weekends, most LNER York to London services leaving around xx:00 leave at xx:59 (non-stop) and xx:02 (slower service).
