After a quick reversal at the currently open station of Princes Risborough*:

Ilmer Halt

* Note - the Wikipedia entry for Princes Risborough suggests that the original station was "a few hundred yards north of the present site". This is misleading, OS maps show that the original station was immediately north of the present site, such that the southern end of the original platforms would have overlapped slightly with the northern end of the current ones. Therefore I suggest that under Rule 4 it doesn't count.

Weedon (2nd station - about 1890 to 1958)

I can only trace a single Weedon station, which was opened in 1838 by the London and Birmingham Railway and closed on 15th September 1958.

The next two stations on the London and Birmingham Railway did have both 1st and 2nd stations....

Blisworth (1st station ... 1845-1853)

I can only trace a single Weedon station, which was opened in 1838 by the London and Birmingham Railway and closed on 15th September 1958.

Map evidence would suggest that at some time between 1885 and 1900 the station was moved about 200 yards, from a location south of what is now the A45 bridge and alongside the canal, to a more spacious site north of the bridge.
