Working it out is easy, it is the first TOC whose delayed train caused an overall delay to your journey.

So if you have A-B-C-D with TOC 1 operating A-B, TOC 2 operating B-C and TOC 3 C-D, and A-B was 10 minutes late but didn't miss the connection, but B-C was 6 minutes late and did, and the following C-D train was an hour late it might feel like you should claim from TOC 3, but it's actually TOC 2 for the entire journey.

It's easy if you're travelling on one train. It's frequently unclear if you're using more than one company's services.

For example, if you make an on time start and run late causing a missed connection, but the connection was cancelled anyway.

If you make an early start on a different operator because your booked service was cancelled with notice, but then miss your connection anyway.

If you make a late start and run significantly late, but still make your booked connection (e.g. your interchange time was 40 minutes), but your booked connection is then delayed en route after making an on time start.

Etc etc

That is an interesting one. Is the passenger expected to check that the train they intended to catch actually ran?

It is clearly not easy for Northern who reject claims where they caused the delay and claim to have sent them on to another TOC who had zero responsibility for the delay. If you contact that second TOC they state they have received nothing from Northern (and I believe them).
