With this, the carer also gets the discount. When using journey planners does the 2nd passenger claim Disabled as well or are the systems clever enough to give 2 discounts for 1 railcard?

Using the National Rail Enquiries website, the answer appears to be that it is clever enough. You could confirm this using the ticket provider of your choice :)

Most journey planners do recognise correctly that a single Disabled Persons Railcard allows up to 2 passengers to travel. My dad has one of these railcards and I'm often helping him with purchasing tickets online for himself and my mum to travel, and all the journey planners I've used for this (GWR, LNER, Thameslink and NRE) have all worked correctly in this regard.

All the journey planners I have used automatically apply the discount for two people.

It's worth noting that the second passenger doesn't need to be a "carer". I hold a disabled persons railcard and it's clear that the second user can be anyone.

All retailers should offer this.

If you are travelling on reserveable trains and want decent seats, you may want to use a retailer that offers a seat selector, but other than that it doesn't matter who you buy from.
