305s and 308s looked pretty much the same to my eyes, along with 304s. Were there any differences that were obvious once you know them?
Similar in appearance yes but the window arrangements were different between the 3 and 4 car units and of course as already pointed out there was a livery change from around 1980 onwards which somewhat changed the appearance factor as well as 302's being part of the equation.

One of the loco hauled single deck peak trains on the usual service I get into Brussels has got two double deck carriages on the end with a driving van trailer. It is quite surreal.
In Luxembourg they regularly operate a double-deck KISS with a single-deck FLIRT. They're even owned by different operators:

On the SZU in Zurich (effectively separate from the mainline), there are a few loco-hauled/pushed trains with 2 double-deck carriages, plus 2 single-deck (one of which is the DSO). Here's an older picture from when they had only one double-deck in the middle (they added a newly built carriage with low-level entry later, and moved them to the end at that point): https://de.wikipedia.org/wiki/SLM_Re_456#/media/Datei:SZU_pendel_456.jpg

I’ve heard that story but not that it was the engine that blew...

Think it happened a few times before the "black boxes" came in... a 90mph run in a Pacer of any sort wouldn't be for the faint-hearted!

You can still find a bit of a mix & match arrangement on the DB loco hauled ICs.

The basic chassis design was tested to 140 and designed to be stable at 125 from what I remember.

I think the same applies to many of the remaining French loco-hauled TER sets, many are a mix of compartment and open Voitures Corail.

Lots of anecdotes suggest that on straight well-maintained CWR at 75, they aren't actually that bad
I agree with those anecdotes and they tally with my own experience... but I've also heard of these occasional whoopsies where a driver in a 158 forgets he has a Pacer on the back. A guard who was in one of these Pacers (on ECS) described it as rather lively!
