I highly doubt the free first class upgrade being refused had anything to do with the children having special needs. Why should anyone get a free first class upgrade? It negates the incentive to pay the premium for the accommodation.

No, but on busy services it is beneficial to all if you do, so as not to deter someone from sitting in your vacant but reserved seat.
