Sorry if mentioned but what happened to 150209 and 150212 was one or both involved in an incident to form 150209? Where is 52209 and 52212?

Are there three Northern 150s with one coach in Centro and one In Network West Midlands livery?

Recently created 150210/150220 and 150226 have two liveries?

They were involved in separate incidents which resulted in both of the 522xx coaches being written off.

150209 and 150212 were both involved in accidents (I think one might have been a level crossing collision?). In each case the 52 cars were written off as beyond economical repair afterwards.

The current 150209 could just as easily have been referred to as 150212 as far as I can see - either unit number could have been chosen, it wouldn't have made much difference...

Should become part of 121 and 127 if the original plan is followed.

I can never remember which one was which but one derailed and hit a bridge support on the Thatto Heath end of St Helens Central (scars still visible) and the other was involved in a collision somewhere on the eastern region. Both damaged cars were stripped and the shells broken up.

Yep, that's absolutely correct. There are three ex-LM 150s in part Centro/part NWM livery; 150210/20/26.

Done some digging and found the report on railwaysarchive.co.uk

St Helens Central was carriage 52209 of 150209, the driver was fatally injured unfortunately. 1988

According to a post in Northern Railways forum carriage 52212 from 150212 was in collision with a civil engineer's crane at Seamer in December 1987 - there doesn't seem to be an online report of that incident.

Presumably if these accidents were to happen now the cars would be rebuilt at enormous cost, as seems to be the current practise. Were these two vehicles formed into three car 150/1 units with LM? Would it not have been simpler to just transfer the units to FGW in that form, rather than splitting and re-forming them?

Yes, they were the centre cars in three car hybrid units 150016 and 150017. 150116 and 150117 have been allocated to Northern, so in the DfTs' complicated, carriage by carriage breakdown of the reallocations, they couldn't all move to FGW together.

There's a surprise...!!

That's only a transitional situation to get them into service quickly, it's already been pointed out in one of the other 150 threads...
Thanks! Ah well, there are so many different threads about the 150 cascade, I thought I might have missed some information somewhere.

I'd have thought rebuilding one of the DMS vehicles with a universal loo, and bringing both vehicles up to the latest accessibility specs as a trial for mass 150 refurbishment could have been an option...
