I'm just wondering what would happen if you had to unexpectedly break a Advanced Ticket journey due to medical reasons with either yourself or the person who you're traveling with

So if a person is traveling a 3 hour journey on an advanced ticket and something happens (Anxiety attack or something similar) that causes the person to want/need to leave the train for whatever reason. What would happen?

Would you need to pay the fair from the BOJ to the destination or worse, pay a fine for ending the journey early.

Of course informing the Manager and/or designated first aider at the station upon arrival as something as serious as having to leave a train would need to be dealt with even if emergency services aren't required.

Understandably a Off Peak ticket would be best to stop this issue as it gives more flexibility of travel

You would hopefully be allowed to break your journey without being charged any extra.

Technically the conditions do allow the train company to charge you the difference between the fare paid and the cheapest walk-up fare that would have permitted the journey you made. It is rail industry policy not to actually do so (except in clear cases of attempting to gain financial advantage)

The train company would not be allowed to charge a new fare; the conditions allow for the difference only. Train companies cannot issue fines under any circumstances, though they can charge fares under certain circumstances (not these) which are so high that people (understandably) consider them to be similar to a fine. In extreme circumstances (again, not applicable here) they can prosecute passengers and, if successful, a court could impose a fine. But you do not need to worry about any of these circumstances.

All tickets allow break of journey unless it is made clear to you that a break of journey isn't permitted.

In the case of a small number Off Peak tickets there are prohibitions on break of journey on the outward portions, and if these are made clear to you when you buy the ticket, then that condition would be contractually binding. However a medical incident would over-ride any contractual matters.
