I have checked the TfL journey planner where fares from NR stations in Southeast (e.g. Bexleyheath) and Southwest London (e.g. Clapham Junction) to indirect London terminals do not have fares.

For example:
Clapham Junction to London Euston. (Avoiding tubes for health concern)
Southeastern stations (except DLR stations) to London Fenchurch Street, London Liverpool Street when Cannon Street is closed.

Does it mean there are fare bugs in TfL fare system, or the fares combinations are just being omitted for simplicity?
Also, will these journey pairs attract TfL-NR Zone 1 "surcharge"?

It isn't a bug - the journeys you note are for journeys that TfL consider most people are unlikely to take so they don't appear in the single fare finder. A fare will be charged (ie not an unresolved journey) although, in some cases, this may include a mixed-mode surcharge.
