The transport undertaking in London has had (at least) four titles since it's inception in 1933 (LPTB, LTE, LRT, TfL).

Are there any current tranport undertakings that have had more and what reason was there for the changes of title ?

Try Coventry:

Coventry and District Tramways (1884)
Coventry Electric Tramways (1895)—switch to electric operation
Coventry Corporation Tramways (1912)—buyout by the city, hence NXC's new "centenary" livery
Coventry Transport (1938)—rebranded
West Midlands PTE (1974, now called Centro of course)—Coventry City became part of WMCC
West Midlands Travel (1986)—deregulation
Travel West Midlands (1996)—bought by NX
Travel Coventry (2002)—separated from TWM
National Express Coventry (2008)—rebranded

Your "possibly more going further back" was too much of a challenge to me!!

Halifax & District Tramways Company - 1882-1937

Halifax Corporation Passenger Transport Committee - 1937-1974

This in turn was divided:-
Passenger Transport Committee "A" Services
owned soley by the Corporation and operating within the Corporation boundaries [thus the "Town" services]

[ii] Joint Omnibus Committee "B" Services
in which Halifax Corporation had a 50% interest - the other 50% divided between the LNER and LMS Railways. This Committee operated the out-of-town services.

I don't claim to remember these personally - rather I am indebted to Geoffrey Hilditch's splendid book "Halifax Passenger Transport 1897-1963" He was General Manager from 1963 to the end in 1974.
