At my medical there was a section on one of the forms to mention any hearing history. Definitely mention it ! as when the doctor looks in your ears they will see scar tissue on your ear drum from where the incision was...

The Photo ID used for your medical is just to establish that it is actually you taking the medical and not some tea total mate you got to stand in for you.
Passport should be fine for medical.
I used my driving license for medical but yes a DL is not accepted at your TOC interview.
The style of the plates were exactly the same but the numbers were different to the ones shown in that link.
It's pretty much that if you get all the answers correct in that online version then you will easily pass the booklet version.

Had my medical just last week. The colour blind test is just this in booklet form.
