Disabled Railcard eligability?

Sorry if this has been covered before.

Thinking of getting my mother in law a disabled person's railcard.
She uses a hearing aid.

Does there have to be any level of proof of deafness for the card to be valid.

I have one myself and have just ordered for my partner who is also a bit on the hard of hearing side and they didnt ask her however she has other disabilitties too. Forum member
