No that was the original design which turned out to be very impractical in operation and was replaced. At Mk5 introduction not all gangways had been modified so key "end coaches" where modified first....all are now the revised design.
You also have to consider than an LNER day coach is likely to do more than one trip per day but the maintenance periodicities are likely to be similar in terms of "operational days" Add in the requirements for multiple locos /drivers per trip and it all adds up.
Certainly at Glasgow stations there is a ticket office "inside" the barriers to allow tickets to be purchased at the destination.....the queue is normally fairly substantial and I always assumed that this was used as sufficient deterrent to deal with anyone who had a genuine opportunity to...
What Tim says is all more or less true but not necessarily the whole story. A lot of work has been put into ensuring the fleet was fit for summer...while I hope luck didn't have too much to do with it, I'm keeping my fingers crossed for a bit more :-)
