    It's useful to have a train waiting at a signal out of the station, as it can move as soon as it gets the signal. If it waits in the station, once it gets the signal it has to go through the dispatch procedure, causing additional delay.

    This also reminds me of Ladbroke Grove. As well as the poor training of a new driver, in both incidents the driver believed the signal was showing a different indication, even though the signal indications were visible for a significant length of time after forming this belief.

    This is not uncommon. As mentioned above, note it, fault it, telephone for emergencies and mention it to your relief.

    Suggested or requested maybe, but signallers are responsible for regulation. If the signaller could show that their decision minimised overall delay, or otherwise followed the regulating policy, they wouldn't get the delay.
