Avanti tickets using health service discount

Does everyone on the booking need to be on the working for health service to be eligible for discounted tickets or can I book for myself and a friend if I only work in health service?

The terms and conditions at the bottom of the page suggest it can be up to 3 accompanying passengers as well as you:
  1. Discount and tickets only valid for sale to registered members. Membership ID should be shown with your ticket.
  2. The discount is available on Avanti West Coast Advance fares on selected Off-Peak services.
  3. Each booking may be made for up to 4 passengers. There are no further discounts for Railcards.

But do accompanying members need to be health service? My friend doesnt work for health service?

Also what card needs to be shown as health service discounts do not issue physical card. Its mentioned on their own website

The terms and conditions don't state that they have to be, so that would suggest it is fine to book for non-Health Service accompanying passengers.

You must have some form of ID to show you work for the NHS (or equivalent)?

Yeah I have NHS card just wondering if they wanted health service website ID card

I'm not sure what "health service website ID" would look like. Do you have a Blue Light Card? I'm sure that would be acceptable too.
