I believe the issue at Cannon Street was that the doors of the train had started to be opened (in those days, not all trains had interlock), and this changed the structural integrity of the unit causing it to weaken and crumple.
Weren't ancient underframes, body corrosion, and just plain lack of crashworthiness in Bulleid's design also factors?
