I bought ticket online on Stagecoach app and forgot to activate it. When ticket inspector asked for ticket I was trying activate my ticket but the metro (run by Nexus) is underground and no internet connection and they gave me penalty fare. Will you get prosecuted for providing false date of birth and address to ticket inspector? What should I do? But I'm sure that I would not do it ever again. should I go back and tell the inspector if I can see them that what I provided was wrong information and tell them the truth. What should I do if I cannot affort the penalty fare

Is this the first time you forgot to activate it? If you have a perfect record of activating before you board the train after purchase then you might be able to demonstrate this as a one-off and pursue the appeals process (although strictly speaking it is your responsibility to have a valid, activated ticket before you board so there is no obligation on their part to grant the appeal).

If you don't have a perfect record, they may have evidence of this and in theory could pursue you for previous times when you activated after boarding. One to think about and if in any doubt, possibly better to just pay the penalty and remember to activate sooner next time.

From their point of view, they will see you as someone who only activates when challenged and hence travelling for free.

well you have been given a penalty fare so you alrerady know if you can afford it or not.
You have complicated things by giving the wrong address etc but if you do not pay then thay can find your correct address very easily through the stagecoach app.
If you do not pay, then they will likely send a letter to the false address. You won't see that letter (as it was a false address) and so the next you are likely to hear is that they have followed it up through the courts. That will become very complicated so your best bet is to just pay the penalty now. It was on a metro so the ticket price is not going to be high and I guess the penalty is about £50? So i guess they are asking for approx £60? If so, then you have 21(?) days to pay and you should save the money or borrow it from a friend and pay in time.
Ignoring this will make matters much worse.

Nexus Penalty Fares and Rights of Appeal do seem to have similarities with National Rail ones at 1st glance (eg £100 reduced to £50 if prompt payment made). Time limit on submitting an Appeal 21 days (presume benefit of doing an Appeal buys time to save up the £50 if Appeal fails)

Being issued with a Penalty Fare is appropriate as you hadn't got a valid ticket when you were inspected. Giving false details is an offence in itself, potentially more serious than the underlying ticketing issue.

If you pay the Penalty fare then I would expect that to be the end of the matter. Matters will get compliacated if Nexus end up having to follow things up because you don't pay.
