It is supposed to be set in the fictional Holby (as you probably know) but they do often show shots of landmarks like the Clifton Bridge and the Floating Harbour.

I am led to believe that many of the "outdoors" shots on the river clearly show that it is set in Bristol. Can't confirm this for myself; never been to Bristol...

Yeah, true that. Not that I watch the programme very often, I hate the sight of blood (real or not).

Holby is set in the west country so yeh, near bristol. in one recent episode the sound of HSTs roaring in the background. as i've got the old series (1,2,3) on DVD, there's also a small part showing a HST roaring through, and like all HSTs from paddington head towards bristol (don't shout at me if they don't!)
