Apologies, honest, to the OP. I find it difficult to post anywhere on the internet without, just a hint of, cynicism and tongue firmly in cheek.
The impression, if one asks a question concerning how much does the test cost, is that one wishes to pay for the test in order to see what the results are on an 'unofficial' basis before taking the test for real as one step in applying for a position with a TOC.
There is enough information on these excellent forums about becoming part of the rail industry to realise that it is all part and parcel of an integrated process. 'Forum Search: medical' should do the trick.

Why wouldn't someone in your mind just take their chances on the free test they'd get if they successfully got through the process? What would they gain from paying to find out something they already knew?
You've also ignored that the OP asked about PTS courses too.
I imagine the OP is looking to get a P-Way job, either with NR or a contractor. Getting into these can be a bit difference to getting into an operational role with a TOC.
