The real danger is on desiros with the GOP left ajar... theres a PA handset in there, which needs no key. Cue chaos :P

Nothing works on the panels without being keyed in on the 15x fleet.
Seems to be common practice on Southern WLL services, with the key usually left hanging lazily from the door.
Oh, that's a regular thing! Sometimes the guard might even be somewhere near the door with the key in, or they might be sitting at the opposite end of the carriage chatting to a mate
158/159s are laughable, some of them just will never lock and need replacing but because there's no real concern about them being accessed (they're not live unless keyed in), then there is certainly no rush and certainly never any disciplinary action!
Leaving it on the "ON" position and walking away with the panel insecure is probably an informal meeting with your manager unless something serious happens.

Depends on what you mean by 'work' - you won't get far with a 153 with a local switch down even if the DKS is switched off - found that some out to my cost
Same on 150/2s and I found out for a similar reason

Yes a far more regular occurrence than it should be! Guards keyed off, but forget to flick the local switch up!
